Kegworth Air Disaster

My copper brother-in-law was on duty in the control room that day.

I worked for many years at BA with one of the cabin crew who survived the crash.
I know exactly where I was when I heard the news on the radio. We all thought it was a terrorist job as it was just a couple of weeks after Lockerbie.

Partly caused by a change in aircraft design.
The previous model would cause the smell of any smoke from the right engine to reach the cockpit.
This model would allow the smoke smell from either engine to reach the cockpit.
The pilot smelled smoke and shut down the right engine, though the problem was with the left one.

Typically though short cuts in pilot training/type familiarisation due to costs were a very significant factor. Tragic and a miracle that more lives were not lost.
