If you haven’t already booked one I used this company in Lutterworth for my Day 2 test on my return from Germany and would recommend them - https://nationwidepathology.co.uk/

Also you have to complete the UK Passenger Locator Form within 48 hours of returning to UK so you’ll possibly have to do this while in Poland. One of the sections of the form requires the details of the Day 2 test booking so take these details with you!!

Little cabbage is out with a serious knee injury sustained in a CL qualifier in July. Scored after 3 mins and then celebrated by jumping up in front of the fans. Sadly his knee buckled and he’s out for 9 months with an ACL.

I presume you are in and out in three days, so you COULD have taken a test here before you went to Poland.

As for the Passenger Locator form for HM Govt, I'd strongly suggest going onto the gov.uk website , and set most of it up before you fly out. You can save it and finish it off in Poland. From memory you need your NHS number, and note it DOESN'T appear on your Covid travel pass.

We did the tests here before going out. Met 3 lads who were pissed in a bar at midnight and were flying at 6am and they'd cocked theirs up and were going to have to re do at the airport. God knows how they got on.
Also met some that didn't even know you needed a passenger locator form and tests to fly home, couldn't believe that.
 
We came home Saturday lunchtime with three guys we met in a cellar bar the night before. Didn't realise they were Roofers until we were on the plane. Top night, chaps! (Did the lateral flow and the Passenger Locator Form the day before. Did the PCR test at home on Monday. What a ball-ache).
